We are required by law to maintain the privacy of your PHI, provide you with this Notice explaining our legal duties and privacy practices, and notify you if a breach of unsecured PHI occurs.

This Notice applies to all Revive & Thrive programs and services, and to all individuals involved in delivering your care, including our employees, volunteers, interns, contractors, and business associates.

A. How We May Use or Disclose Your Information

We may use and disclose your PHI without your written authorization for the following purposes, as permitted or required by law:

  • Program Services: We use your information to deliver program services effectively to communicate with you, tailor support, and coordinate with partner organizations or service providers.

  • Operational Purposes: We use data for internal program evaluation, quality improvement, staff training, auditing, and general operations.

  • Appointment Reminders/Service Notifications: We may contact you with reminders, updates, or announcements about services or events.

  • Family or Care Partner Communication: With your consent (or in accordance with applicable guidelines), we may share relevant information with your chosen family/care partner.

  • Legal and Safety Requirements: As required by law, we may disclose information to authorities or relevant organizations.

  • Breach Notification: Should there be an unauthorized breach of your personal information, we will notify you in accordance with applicable law.

  • Fundraising Communications: We may use contact and demographic information to inform you about ways to support the Revive & Thrive.

B. When We Will Not Use or Disclose Your Information

Except as described above or as permitted/required by law, we will not use or disclose your personal information without your written consent. You may revoke any authorization at any time in writing.

C. Your Information Rights

  • Request Restrictions: Request limits on how we use or share your information, although we may not always agree to restrictions.

  • Request Confidential Communications: Ask that communications be made via an alternate method or location.

  • Inspect and Copy Your Information: Request access to your personal data that we've collected. A reasonable fee may apply.

  • Request Amendments: If you believe your information is inaccurate/incomplete, you may submit a written request to amend it.

  • Request an Accounting of Disclosures: Ask for a list of disclosures we have made about your data, except for disclosures permitted by law.

  • Paper or Electronic Copy of This Notice: You are entitled to receive a copy of this Notice in paper or electronic form at any time.

D. Changes to this Notice

We may update this Notice as needed. Any new version will apply to all current and future information we hold, regardless of when it was collected. The current version will be available at our service locations and posted on our website.

E. Complaints

If you believe your privacy rights have been violated, please contact our Privacy Officer immediately. If you remain unsatisfied, you may file a complaint with the appropriate supervisory authority or oversight body.
