In order to qualify, an individual must be:

  • Diagnosed and currently in treatment for cancer, heart failure, heart disease, or Diabetes. We also provide a more limited meal service option for individuals who have had an organ transplant within 60 days of application.

  • Unable to prepare meals for a short term due to the medical crisis. Revive & Thrive is a short-term meal service focused on serving individuals whose ultimate goal is to recover from their illness and resume independent shopping and cooking. We provide a maximum of 24 weeks of healthy meals per household.

  • Motivated to eat nutritious, anti-inflammatory food that is aligned with the DASH and Mediterranean diets. Our meals are 85% vegetarian; each meal includes fresh vegetables and lean protein to support the healing process.

  • To receive delivered meals, the client must live in the cities of Grand Rapids, East Grand Rapids, Wyoming, Grandville, Kentwood, or Walker. For those living outside of these areas, meal pick-up is available at the Revive & Thrive kitchen (1815 Hall St SE) on Wednesdays from 6-7pm. Clients must make arrangements to pick up meals at the kitchen, if living outside the delivery area.

  • Not receiving hospice care.

  • Not diagnosed with Celiac disease or a severe food allergy.

  • Not on a soft-foods or liquids diet.

  • Stably housed and able to safely store and reheat meals.

* Please note that Revive & Thrive Project does not have an age or income restriction to receive meal service.


How to Apply

Fill out our New Client Referral Form to send an encrypted referral via Microsoft Forms.

Our client care team will contact you within 5 business days to confirm your eligibility, arrange enrollment, and answer any questions you have.

Once enrollment paperwork is completed, you will be provided a start date for meal service or added to the waiting list if one is in effect.
